Office Information:


*New Patients: (Welcome!) *

All new patients are required to bring the following documents to their
initial visit:

  • All health plan cards
  • Photo ID (Driver’s license, State Issued ID, or Student ID)
  • Cash or Credit card for co-pays (checks not accepted)

New patients are required to check-in 30 minutes prior to their scheduled appointment time to fill out New Patient Forms and verify insurance. Patients unable to check in 30 minutes prior to their appointment are
invited to download our new patient forms under “Patient Documents” and complete them prior to visiting.

A list of all your medications, prescriptions, over-the-counter meds, vitamins, and any herbal supplements is also greatly appreciated.



*Established & Returning Patients:*

All established patients are required to bring the following documents to visits:

  • Cash or Credit Card: Co-pays are due at the time of service
  • Photo ID (Driver’s license, State Issued ID, or Student ID)
  • Health Insurance cards if you have a new policy.

Patients who have not been seen at our office within 3 years will be considered new patients re-establishing their care. Please see the new patient section.


*Late Arrival & No Show Policy *

If you are more than 15 minutes late for your appointment time, you may be required to reschedule to the next available opening.

We as a practice understand that life can get in the way and appointments are sometimes missed. This is why we allow a maximum of 5 no shows per patient. More than 5 no shows on file will result in requiring management approval to reschedule.

If you have a balance due, you may pay over the phone with a credit card or send a check or money order by mail. We do not keep credit card information on file unless the patient is on an agreed payment plan. Any questions related to billing or claims can be relayed to our billing department (480) 427-3400.



If you were referred to us by your primary doctor for a medical condition, please ask them if you need a referral before scheduling an appointment with us.

If one of our physicians refers you out to another specialist, please allow us up to 4 business days to process.

If you are seeing a specialist that is unrelated to OBGYN, you will need to obtain a referral from your primary care doctor. If you don't have a primary doctor, please refer to our links page to see the list of doctors we recommend.


*Release of Records*

For copies of your medical records please complete a Release of Records form. You can obtain this form under “Patient documents” and fax it to us, or visit the office to complete it in person. Please allow 3-5 business days for us to prepare your documents. There may be a charge for records depending on the total quantity of pages, please contact our office for more information.


*Guest Policy: (MASK REQUIRED)*

Scheduled patients are allowed *ONE* additional guest to be present during their appointment and ultrasound. We currently do not allow more than one guest per patient to ensure our facility remains minimally occupied during the ongoing pandemic. CHILDREN: With school out of session, we understand there may be challenges to secure childcare for your upcoming appointment. We always allow patients to bring children to their visit if absolutely necessary, but we kindly ask every effort be made to AVOID bringing your children until further notice.
