All new patients will be required to bring the following:
- All health insurance plan cards
- Photo ID
- Cash or Credit card for co-pays (checks are not acceptable)
You will be asked to complete new patient forms, which you can download online if you wish to do so.
A list of all your medications, prescriptions, over the counter meds, vitamins, and any herbal supplements.
Established patients will be required to bring the following
- Cash or Credit Card - Co-pays are due at time of service
- Photo ID
- Health Insurance cards if you have a new policy.
If you are more than 15 minutes late, you must reschedule your appointment.
Any cancellations or reschedules must be called in 24 hours prior to your appointment time in order to avoid fee.
If you have a balance due, you may pay over the phone with a credit card or mail in a check or money order. We do not keep credit card information on file for security reasons. Any questions you might have about billing you can contact our office.
If you were referred to us by your primary doctor for a medical condition, please ask them if you need a referral before scheduling an appointment with us.
If one of our physicians refers you out to another specialist please allow us up to 4 business days to process.
If you are seeing a specialist that is unrelated to OBGYN, you need to obtain a referral from your primary doctor. If you don't have a primary doctor, please refer to our links page to see the list of doctors we recommend.
Release of Records
For copies of your medical records please complete a Release of Records form and give us 3-5 business days to prepare your paperwork. There may may be a charge for records, excluding AHCCCS patients.
